Help Through Transition Process
Transition Age Students
Transition planning consists of a coordinated set of activities that will help students move successfully from high school to adult life. Transition services are individualized and based on each student’s needs, preferences and interests. By the time a student reaches age 14, transition should drive the IEP. Each year, The IEP team (which includes the student and the parents) will identify the student’s wishes for postsecondary education or vocational training, competitive employment, independent living, and community participation after high school. The IEP must then include the coursework, annual goals and transition services necessary to work toward the student’s future plans. The student must be invited to any IEP meeting where postsecondary goals and transition services needed to reach those goals will be considered. With parent or student (if 18) permission, representatives from other agencies who may be responsible for providing or paying for transition services in the IEP must also be invited to the IEP meeting.
